Home Health Plus is a physician guided care for patients who are homebound. It is designed to meet the needs of patients recovering from surgery or illness; patients recovering from stroke; those who are disabled or handicapped; elderly in need of medical assistance; and patients with chronic disease such as Cancer, High Blood Pressure, Diabetes, etc. The Home Health nurse becomes another pair of “eyes and ears” for the physician. Contact between the patient’s personal physician and the home health nurse is on-going once a patient is admitted by Home Health Plus.
The mission of Home Health Plus is to participate as an active part of the community, in providing and continuously improving the home health care needs of the patient by delivering value driven, high quality, compassionate care.
